Rescue Adoption Application
Incomplete applications WILL NOT be processed.
Completion of this application does not guarantee adoption of a rescue animal.
A few requirements before you can adopt. You must:
Be at least 21 years of age
Make sure that everyone in your household knows and agrees your new friend can live there
Have a valid identification with your current address
In the event that you need to return your adopted rescue dog, we REQUIRE that you bring the dog back to our facility for re-homing. Adoption fees are non-refundable.
